Let’s talk about a tale of two employers.

Employer A is a nice guy. He trusts his Pharmacy Benefit Manager (PBM) implicitly. He signs the contracts, pays the bills, and assumes that if there’s a rebate to be had, he’s getting it. He doesn’t look too closely at the "spread": that murky gap between what he pays the PBM and what the PBM actually pays the pharmacy. He thinks he’s doing fine.

If your contract doesn't explicitly state that you get every penny of those manufacturer rebates, you’re essentially leaving a bag of cash on the sidewalk.

  • Translation: If your PBM refuses to show you the rebate flow, they are likely non-compliant with new fiduciary standards.
