If you’re a business owner or an HR leader with 25 to 300 employees, you know the annual "renewal dance" all too well. Your broker walks in, sighs deeply, and hands you a piece of paper showing a 15%, 20%, or: heaven forbid: a 30% increase in your health insurance premiums.
You ask why. They mention "market trends" or a "bad year for claims." You pay the bill because you have to stay competitive, but deep down, it feels like you're throwing money into a black hole.
For years, small and mid-sized businesses (SMBs) were stuck. You were either "Fully Insured" (paying a flat, high rate to a carrier who kept every penny of profit) or "Self-Insured" (taking on massive risk that only Fortune 500 companies could handle).
There was no middle ground. Until now.
Enter Level-Funding. It’s the bridge between the two extremes, and it’s quickly becoming the secret weapon for SMBs looking to claw back control over their healthcare spend. In this edition of our In Plain Benefits series, we’re stripping away the jargon to show you exactly how level-funding works and why it might be the ROI-driven solution your budget has been waiting for.
To understand level-funding, you first have to understand the two traditional ways companies pay for healthcare.
Level-funding is the hybrid. It gives you the predictable monthly cost of a fully insured plan with the "upside" potential and data access of a self-funded plan.

The biggest fear SMBs have about moving away from traditional insurance is volatility. You need to know exactly what is leaving your bank account every month to manage cash flow.
With a level-funded plan, your monthly payment is "leveled." Your broker and the carrier look at your group's health and determine a maximum cost for the year. This total is divided into 12 equal monthly payments.
This payment typically covers three things:
Because the payment is fixed, it feels just like the traditional insurance you’re used to. You don't have to worry about a "spike" in claims ruining your February budget.
This is where level-funding starts to feel like a secret weapon. In a traditional fully insured plan, if you pay $500,000 in premiums but your employees only use $300,000 in healthcare, the insurance company says "Thank you very much" and keeps the $200,000 difference.
In a level-funded plan, that $200,000 doesn't automatically belong to the carrier.
Because you are technically "self-funding" (with a safety net), any money left over in your Claims Fund at the end of the year can be returned to you. Depending on your contract, you might get 50% or even 100% of that surplus back as a refund or a credit toward next year’s costs.
Imagine getting a $40,000 check back at the end of the year because your team stayed healthy. That’s money that can be reinvested into financial wellness programs, salary increases, or new equipment.

One of the most frustrating parts of traditional insurance is the lack of data. When your rates go up 20%, the carrier rarely gives you the "why." They might say "high claims," but they won't tell you if those claims were for things that could have been prevented.
Level-funding opens the "black box." Because you are paying the claims directly, you get access to detailed (but anonymized) reporting.
Why does this matter?
Data is the only way to move from being a "passive payer" of insurance to an "active manager" of your healthcare costs.
Most people don't realize that fully insured plans are subject to various state taxes and mandates. These "hidden costs" can add 2% to 5% to your premium before you’ve even paid for a single doctor’s visit.
Level-funded plans fall under federal ERISA guidelines rather than state-specific insurance mandates. This allows you to:
While the upside is huge, level-funding isn't a "magic wand." It requires a healthy dose of transparency and a willingness to look at your benefits as a multi-year strategy rather than a one-year transaction.
Typically, the "sweet spot" for level-funding is an SMB with:

The "renewal dance" is exhausting. You get a 20% increase, your broker shrugs, and you sign the check. But what if you could get a refund when your employees stay healthy?
Level-funding is the bridge between traditional insurance and self-funding. It’s the "secret weapon" for companies with 25-300 employees who want the upside of the big guys without the scary risk.
Here is why it’s winning in 2026:
1️⃣ FIXED MONTHLY BUDGETS
Stop worrying about "claims spikes." You pay one set amount every month, just like a traditional plan. It covers admin, your safety-net insurance, and your claims fund. Predictability is king for cash flow.
2️⃣ THE END-OF-YEAR REFUND
In a traditional plan, if your employees don't use their insurance, the carrier keeps the profit. In a level-funded plan, if there’s money left in your claims fund at the end of the year, YOU get a check back. It’s your money: why should they keep it?
3️⃣ TOTAL DATA TRANSPARENCY
Stop flying blind. Level-funding gives you the "receipts." You can see exactly where your healthcare dollars are going (anonymously, of course). When you have the data, you can make actual strategic moves to lower costs instead of just guessing.
4️⃣ BYPASSING HIDDEN TAXES
Fully insured plans are loaded with state premium taxes and mandates. Level-funded plans (under ERISA) avoid many of these "hidden fees," often saving you 2-5% right off the top before a single claim is even filed.
Stop leaving money on the table. If you have 25+ employees, you should be looking at this.
